Comprehensive Case Management Services

Community Re-Entry’s Comprehensive Case Management Services has collaborated in a unique and innovative way with Center for Families and Children and many local and state community justice organizations to establish the Greater Cleveland Integrated Re-Entry Project (GCIRP).  Through this endeavor, Community Re-Entry is assisting incarcerated persons and their families in making effective transitions for themselves and their communities by offering 3-6 months of pre-release case management services inside several state institutions and 6 months of post-release community-based intensive case management for clients.  This project takes into consideration the importance of family and support persons by including the client’s support network in all wrap-around services offered to the client.  To be eligible for this program, the individual must be an adult that will be residing in Cuyahoga County upon release.  Currently the four case managers with CCMS have worked with 104 clients and dozens of support network persons.

By utilizing a broad Service Provider Network to provide employment readiness and placement, housing referrals, education, physical health and dental services, financial literacy instruction, drug and alcohol assessments and treatment, clothing, transportation and financial assistance, our case managers are able to assist clients with most needs upon release.  

 

Furthermore, case managers are addressing problem solving and coping skills, acting as a sounding board for developing new ways of thinking and acting and are helping clients to face the demands of life.  Case managers promote and support their clients by encouraging strengths and raising confidence and helping clients to have realistic expectations.   

 

In addition, GCIRP is raising awareness of the challenges and barriers associated with successful transitions and advocating for systemic change through community education and promotion.
